Community HealthChoices (CHC) is Pennsylvania's Medicaid long-term care program. Through its "Services My Way" self-direction option, a Medicaid member can hire, train, and pay their own caregiver — including an adult child or other relative — instead of using an agency.

What is Community HealthChoices and Services My Way?

Community HealthChoices (CHC) is Pennsylvania's mandatory managed-care program for long-term services and supports (LTSS). It serves adults 21 and older who are "dual eligible" for both Medicare and Medicaid, as well as adults with physical disabilities who need a nursing-facility level of care but want to stay in their own home. Instead of paying for care service-by-service, the state pays one of three managed care organizations (MCOs) to coordinate each member's health care and in-home supports.

Personal assistance services — help with bathing, dressing, meals, mobility, and other daily tasks — are one of the core CHC home and community-based benefits. Normally a home-care agency assigns you a worker. But CHC also offers a self-directed option called Services My Way, where the participant (or a representative) becomes the employer of their own worker and decides who provides their care.

Services My Way is built on the "participant-directed" or "consumer-directed" model. Because the participant hires and trains the worker directly, no formal home health or nursing certification is required. An adult child who has quietly been caring for an aging parent for years can finally be paid for that work — as long as the parent qualifies for CHC and the child is not a spouse or the participant's legal guardian.

Under Services My Way, the participant is the "Common Law Employer." A statewide Financial Management Services (FMS) vendor — Tempus Unlimited for CHC members — acts as the fiscal agent. Tempus issues paychecks, withholds taxes, files payroll paperwork, runs background checks, and manages workers' compensation. It does not choose the worker, set the schedule, or supervise the care; those decisions stay with the participant and their CHC service coordinator.

Community HealthChoices eligibility requirements

To use Services My Way, the person receiving care must qualify for Pennsylvania Medicaid (Medical Assistance), meet the clinical (nursing-facility) level-of-care standard, and be enrolled in CHC. The caregiver does not need to meet income or asset limits — only the participant does.

Age 21 or older
Community HealthChoices serves adults 21 and older. Younger Pennsylvanians who need long-term supports are served through other programs, not CHC.
Pennsylvania Medical Assistance (Medicaid) eligibility
The participant must be financially eligible for PA Medicaid. For 2026, the long-term-care income limit is roughly $2,900-$2,980 per month for an individual (about 300% of the federal SSI benefit rate); the asset limit for a single person is generally $2,000. A County Assistance Office (CAO) makes the final financial determination.
Dual eligible or physical disability
CHC covers adults who have both Medicare and Medicaid ("dual eligible") and adults with a physical disability. People receiving services through the Office of Developmental Programs for an intellectual or developmental disability are enrolled elsewhere, not in CHC.
Nursing-facility clinical level of care
To receive in-home personal assistance (and therefore Services My Way), the participant must be assessed as needing a nursing-facility level of care — meaning they would otherwise qualify for a nursing home but choose to stay at home. This clinical determination is made by the Independent Enrollment Broker (IEB).
Pennsylvania residency
The participant must live in Pennsylvania, and care must be provided in their home or community setting in the state. The worker can be a relative who lives with them or nearby.
Enrolled in a CHC managed care organization
The participant must enroll with one of the three statewide CHC-MCOs — AmeriHealth Caritas / Keystone First, PA Health & Wellness, or UPMC Community HealthChoices. The chosen plan's service coordinator authorizes the personal assistance hours.

Who can — and cannot — be paid under Services My Way

Services My Way lets the participant choose their own worker, and Pennsylvania allows most relatives to be hired. The key exclusion is the participant's spouse (and their legal guardian), who cannot be paid under this program.

Services My Way pay, hours, and overtime

Under Services My Way the participant and worker agree on an hourly rate, within a floor (Pennsylvania minimum wage) and a maximum set by the state. Recent budget investments have pushed wages toward and above $15 per hour, and the number of authorized hours depends on the participant's assessed needs.

Hourly pay

In Services My Way, the Common Law Employer (the participant) and the Direct Care Worker agree on the hourly wage. It must fall between Pennsylvania minimum wage and a maximum rate that varies by the participant's county and, as of 2026, by which CHC managed care plan they are enrolled in. Because of investments in Governor Shapiro's 2025-26 budget (retroactive to January 1, 2026), participant-directed direct care worker rates now support a roughly $15-per-hour wage, with maximum county rates running higher — most workers land in the $15 to $19 per hour range, and some high-cost regions allow more. PA Health & Wellness generally offers the highest maximums and AmeriHealth Caritas the lowest. Tempus Unlimited (the FMS vendor) provides plan-specific pay-rate calculators, issues W-2 paychecks, and withholds federal and state taxes.

Hours and scheduling

Authorized hours are based on the participant's needs assessment and set by the CHC-MCO service coordinator in the person-centered service plan. Many participants receive roughly 20-40 hours of personal assistance per week; those with higher needs may be authorized for more. The participant can split the authorized hours among more than one worker — for example, two adult children covering different days.

Overtime rules

Federal overtime rules apply through the FMS vendor: a worker who exceeds 40 hours in a single workweek for one participant is generally owed overtime at 1.5x. Pennsylvania caps and manages overtime under the participant-directed model, so families who need more coverage often schedule two workers to stay within each worker's hours rather than pay overtime.

How to apply for Community HealthChoices and Services My Way

  1. Confirm or apply for Pennsylvania Medical Assistance (Medicaid). If the participant is not yet enrolled, apply through COMPASS (compass.dhs.pa.gov), by phone at the Consumer Service Center (1-866-550-4355), or at your local County Assistance Office.
  2. Call the Independent Enrollment Broker (IEB) at 1-877-550-4227 to start long-term-care enrollment. The IEB is the front door for CHC home and community-based services.
    • The IEB helps you apply for LTSS and submits paperwork to the County Assistance Office
    • They schedule a level-of-care assessment in your home, hospital, or nursing facility
    • A nurse or assessor confirms you need a nursing-facility level of care to receive in-home services
  3. Choose a CHC managed care organization — AmeriHealth Caritas / Keystone First, PA Health & Wellness, or UPMC Community HealthChoices. If you do not pick one, the state assigns you a plan; you can switch later.
  4. Ask your CHC service coordinator to enroll you in Services My Way (the participant-directed option) rather than agency-directed personal assistance. Tell them you want to hire and direct your own worker.

Can my spouse be paid to care for me under Services My Way?

No. In Pennsylvania, a spouse is treated as a "legally responsible relative," and legally responsible relatives cannot be paid as Direct Care Workers under Services My Way or the broader Community HealthChoices participant-directed model. The participant's legal guardian is also excluded. Pennsylvania, like most states, assumes spouses already provide care to one another, so Medicaid dollars are reserved for other family members and helpers. The good news is that almost everyone else can be paid: adult children, siblings, grandchildren, nieces, nephews, in-laws, and even friends or neighbors are all eligible to be hired as the participant's worker. If you specifically need a spouse to be paid, the participant may want to explore VA Veteran-Directed Care (if they are a veteran), which can allow spouses to be paid in some situations. But under standard CHC Medicaid rules, a spouse cannot receive a Services My Way paycheck.

How much does Services My Way pay in 2026?

Under Services My Way, the participant (as the "Common Law Employer") and the Direct Care Worker agree on an hourly wage, which must fall between Pennsylvania's minimum wage and a maximum set by the state. Thanks to investments in Governor Shapiro's 2025-26 budget — retroactive to January 1, 2026 — participant-directed direct care worker rates now support roughly a $15-per-hour wage, and most workers earn between about $15 and $19 per hour. The exact maximum depends on the county the participant lives in and, for the first time in 2026, on which of the three CHC managed care plans they are enrolled in: PA Health & Wellness generally offers the highest maximums, while AmeriHealth Caritas offers the lowest. Workers are W-2 employees, so income taxes, Social Security, and Medicare are withheld from each paycheck.

How long does it take to get approved and start getting paid?

Plan for roughly two to three months from start to first paycheck, though it varies. If the participant already has Pennsylvania Medicaid and only needs the long-term-care piece, the Independent Enrollment Broker typically schedules the in-home level-of-care assessment within a few weeks; after that, enrolling in a CHC plan and setting up Services My Way with the service coordinator takes another few weeks. If the participant is not yet on Medicaid, add 30-90 days for the County Assistance Office to process financial eligibility. Onboarding the worker through Tempus adds time too: budget four to six weeks for the criminal background check and abuse-registry clearances alone, since a worker cannot be paid until those clear. You can speed things up by gathering documents early — proof of residency, ID, Social Security cards, and income and asset records for the participant, plus identification for the worker.

What training or certification does the caregiver need?

One of the biggest advantages of Services My Way is that no formal certification or license is required. The Direct Care Worker does not need to be a Certified Nursing Assistant (CNA), Home Health Aide (HHA), or any other licensed professional. Because this is a participant-directed program, the participant (or their representative) trains the worker on exactly the tasks they need — bathing, dressing, transfers, meals, medication reminders, and so on. There is no clinical skills exam. What the worker does have to complete is enrollment paperwork through Tempus Unlimited: a criminal background check, Pennsylvania child-abuse and elder-abuse registry clearances, federal I-9 and W-4 forms, and a signed pay-rate agreement. This makes Services My Way especially welcoming to relatives who have been informally caring for a family member for years and simply want to be paid for that work.

Can an adult child be paid to care for a parent?

Yes. An adult child (18 or older) is one of the most common Direct Care Workers under Services My Way. As long as the parent qualifies for Community HealthChoices — meaning they are 21 or older, financially eligible for Pennsylvania Medicaid, and assessed as needing a nursing-facility level of care — the adult child can be hired, trained by the parent, and paid through Tempus Unlimited. The only close relatives Pennsylvania excludes are the participant's spouse and legal guardian. Adult children, stepchildren, siblings, grandchildren, and in-laws are all eligible. Many families use this to formalize care that was already happening: an adult child who moved home to help an aging parent can now receive a real paycheck, tax withholding, and workers' compensation coverage for the same work. The child can even continue living in the parent's household while being paid.

What is Tempus Unlimited and what does the FMS vendor do?

Tempus Unlimited is the statewide Financial Management Services (FMS) vendor for Community HealthChoices participant-directed services; it took over CHC in 2022 (Public Partnerships / PPL still handles the OBRA waiver and Act 150 program). In the Services My Way model, the participant is the legal employer of their worker, but handling payroll and taxes on your own would be a burden — so Tempus acts as the fiscal agent. It issues paychecks, withholds and files federal and state payroll taxes, processes timesheets, runs the required criminal background and abuse-registry checks, and manages workers' compensation. Importantly, Tempus does not choose your worker, set their schedule, or supervise the care — those decisions stay entirely with the participant and their CHC service coordinator. Tempus also provides online tools such as pay-rate calculators and an employee self-service portal where workers can view their pay stubs.

How do I choose between the three CHC managed care plans?

Every Pennsylvania county is covered by all three Community HealthChoices managed care organizations: AmeriHealth Caritas (branded Keystone First Community HealthChoices in the five southeastern counties around Philadelphia), PA Health & Wellness, and UPMC Community HealthChoices. All three cover the same core Medicaid long-term-care benefits, including Services My Way, so you cannot go wrong on the basics. The differences show up in the "extras": each plan offers different supplemental benefits, has different provider networks in your area, and — new for 2026 — sets different maximum Direct Care Worker pay rates. If paying your worker as much as possible matters to you, compare the plans' maximum rate sheets (PA Health & Wellness tends to be highest). If you have a preferred doctor or specialist, check which plan includes them. Will using Services My Way affect the participant's other Medicaid or Medicare benefits?

No. Choosing Services My Way is simply choosing how personal assistance is delivered — you self-direct your own worker instead of using an agency — and it does not reduce or change the participant's other coverage. They keep their doctor visits, hospital care, prescription drugs, durable medical equipment, and every other benefit in their Community HealthChoices package. For participants who are "dual eligible," Medicare continues to cover acute and short-term care (hospital stays, physician visits, short-term skilled home health), while CHC Medicaid covers the ongoing long-term personal assistance that Services My Way provides. The two programs coordinate through the CHC managed care plan but do not cancel each other out. The only thing Services My Way changes is who employs and directs the worker — putting the participant, rather than an agency, in control of their day-to-day care.
